The leech has developed a second mouth at the back of its throat that it uses for the specific purpose of speaking.
Speech:
Through heightened telekenetic abilities or otherwise, the leech can speak to other creatures in a language that they understand. In this particular case, the leech uses its second mouth and developed voice box to speak the common language.
Glamour:
Through use of developed telekenesis the leech can weave an illusion around its body, making it appear to be human to any creature that it encounters. This fae-like glamour is not broken until the leech decides to end it, or until it becomes distratcted or incapacitated.
Anthropomorphic Form:
Due to its age and abilities, the leech's body has grown and evolved into a stance that is wildly different from the common leech monster. It has become bipedal and its form much closer to that of a human.
